Tooth accidentally pushed in maxillary antrum during 3^rd molar extraction, what will you do?

Accidental displacement of a maxillary third molar or root into the maxillary sinus (antrum) is a recognized complication of extraction. A displaced tooth is a foreign body within the sinus and generally requires surgical retrieval, rather than being deliberately left in place.

A retained tooth/root can interfere with normal sinus physiology and may predispose to maxillary sinusitis, infection, mucosal inflammation, or other complications.

Depending on its location, size, and clinical circumstances, retrieval may be performed through the extraction socket/endoscopic approaches or, in selected cases, a Caldwell-Luc approach. Imaging such as CBCT can help localize the displaced tooth and plan retrieval.
